U.S. University Host Application Interest Form

Do you represent a U.S. higher education institution with graduate-level programs or can offer graduate level programs through a consortium?
Are you willing to look for faculty members at your institution to serve as academic advisors for the visiting JFDP Fellows in the Spring semester?
Can your institution allow JFDP Fellows to informally observe a minimum of four courses in their relevant academic fields? (official enrollment or registration in courses is not allowed under program policy)
